Academic/Professional Requirements for
KOTF Scholarships

· Demonstrate outstanding scholastic achievement (3.0 or higher GPA)

· Demonstrate a record of leadership, service, and membership in professional Occupational Therapy organizations but not limited to KOTA, AOTA, SOTA, etc.

· Currently enrolled in a higher education program in the state of Kentucky.

 
Application Requirements

Applicants should submit the completed application form and the following supporting data:

· Two sealed letters of reference

· Personal essay—2 to 3 pages describing your involvement in Occupational Therapy, career aspirations, extra-curricular activities, financial need, leadership and/or other pertinent information you would like to share with the scholarship committee.

· Official transcript

Submit application and supporting documentation postmarked by June 1, 2008.  Application packets will not be returned to applicants.  Decisions will be made by July 15, 2008.

           An award has been established in honor of Dr. Deborah R. Labovitz, an outstanding and highly recognized occupational therapy educator, practitioner, administrator, researcher and mentor from New York.

Criteria for the Dr. Deborah R. Labovitz Faculty Award:
The award recipient must be a full-time faculty member from any of the occupational therapy educational programs within the Commonwealth of Kentucky who is a member in good standing of the Kentucky Occupational Therapy Association (KOTA) and who has demonstrated one or more of the following accomplishments within the year prior to application:

· Lead investigator or researcher in area of innovative occupational therapy practice.

· Leader or facilitator of course work to promote cultural diversity and/or outreach.

· Innovator for work/study student enrollment or other unique educational involvement.

· Leader in field of occupational therapy at state and national levels.

· Author of article or book to promote public awareness of occupational therapy and therapist.
